Жалобу подавала через официальный сайт LOT - там есть функция обратной связи, указывала свои данные, номер рейса и т.п. Так же там можно прикреплять файлы, и писать саму жалобу в окне для текста. По первой жалобе прикрепляла электронные билеты - старые и новые, по второй то же самое + подтверждение бронирования отеля, и скриншоты двух поездок убером. На еду не нашли чеки.
По первой компенсации сама жалоба:
Good afternoon!
I had a flight from Kiev to Amsterdam with a stop in Warsaw on ________.
The 1st flight Kiev - Warsaw №_____ was delayed. That’s why I missed my 2nd flight from Warsaw to Amsterdam (№____). That flight should have been at 07:20 a.m.
Your company gave me a new ticket for that flight - №_____ at 12:10 p.m.
I had to wait at the airport several hours. Finally I arrived to Amsterdam at 2:15 p.m. instead of 9:25 a.m.
It is 4 hours 40 minutes later than I planned.
I kindly ask you to pay me a compensation according to The Flight Compensation Regulation 261/2004.

Good afternoon!
I should have had a flight from Amsterdam to Kiev with a stop in Warsaw on _________.
I was in the first plane to Warsaw when it was getting ready to take off.
But there were some problems and we couldn’t take off during several hours. Finally the pilot or a flight attendant, I don’t remember who of them, told to passengers:
«Who missed their flights from Warsaw to anywhere should come back to the airport in Amsterdam».
They sent a bus to take us from the plane.
I and other people who had connecting flights in Warsaw went to the bus and were returned to the airport.
But when I came back to the airport, nobody was at customer service. I couldn’t find where to take my baggage. Finally I called to airlines but nobody knew about the situation. I was refused in accommodation. But I got a letter with new tickets to Kiev for the next day.
I had to book and pay for a night in a hotel, a taxi from the airport to the hotel and from the hotel to the airport. Also I had to pay for my dinner and breakfast.
I ask you to pay me a compensation according to The Flight Compensation Regulation 261/2004 plus compensation for my hotel, 2 taxi rides and a dinner with a breakfast.
My receipts are attached.

Потом через месяц где-то приходит ответ на e-mail по каждой жалобе отдельно. В письме текст, что решили выплатить компенсацию такую-то, и ссылка. По ней нужно перейти и указать свои данные и реквизиты банковского счета.
Потом приходят деньги на счет.
Никакой как таковой переписки с сотрудниками авиакомпании не было.
На каждого пассажира нужно отдельную жалобу подавать.
Ну и я специально не указывала, сколько денег хочу. По второй компенсации рассчитывала на меньшую сумму, откуда они столько насчитали - не знаю.
